Do you have to go pesh for accidentally removing hair? For example cleaning the inside of your nose with a tissue and hair being scraped out. Or beard hair being pulled by accident.

Daas's humble opinion is no, you do not need to go Pesh for accidentally removing hair. It all comes down to intention.

Hair falls off your body naturally day to day e.g. when you shower, when you comb your Kesh. But we keep our hair and allow it to grow, as it is Guru's will, and we accept it.

If you had intended to dishonour your hair, with intent, then yes, but in this situation, I would say no.

No, only since singh sabha movement. Teja Singh Bhasuria, Gyani Dit Singh, etc.

That stuff was also only confined to certain groups.
